Root Cause: Failure of a defective transformer in a UPS system B distribution panel which resulted in a loss of power to certain critical system components in the data center. Apparently the system did not automatically failover to the redundant system A power as designed because system A power distribution panel breaker was left in a non-redundant state for some unknown reason.
