This is the kind of event that needs the Five Whys asked and a deep dive into what processes were not followed or what processes need to be put in place to avoid a similar situation. I wonder if this is a US Consumer Safety Board worthy investigation. Electricity is scary stuff if you make mistakes. I hope the team injured are able to recover fully.
